Conditions apply for Trust General Health Card

1) One card can be used for the whole family.

2) Trust Card can be used any number of times. With this card, patients can avail services in all the hospitals mentioned in the list.

3) For smooth and trouble free service from the hospitals, patients should necessarily intimate the hospital contact people before going. Trust will not be responsible if patients fail to do so.

4) After showing the Trust Card at the hospital counter, patients should take back the card and keep it with them for future use.

5) Usha Rani Memorial Trust does not force any patient to go to Trust tie-up hospitals. It is completely the patient’s decision.

6) Usha Rani Memorial Trust does not charge anything from the patients or from the hospitals for sending referral cases. Trust provides discount directly to the patients from the hospitals.

7) Trust will not be responsible if hospital administration changes the discount rates at any time. Sometimes patient discounts also depend on their case.

8) Only patients with Trust General Health Card are eligible for all the discounts from hospitals. No discount will be provided if card is not produced.

9) Patients going to hospitals along with the Trust Card should necessarily carry a valid ID proof and show it at the hospital counter.

Our trust conducted medical camp on 25 - 9 -2014. At IDPL Colony - Sita rama kalyana mandapam -Chintal - Hyderabad. We conducted camp only 2hrs timing ( 10 a.m to 12 p.m ).

Dr.Karena supported to this camp. More than 50 patients attend to this camp.

Our trust provided free medicines to the needy.
